Based on our records, your account information matched a record with a master death database. In order to continue receiving your benefit services, you must submit documents to prove your identity AND that you are alive. You must mail all documentation within 30 days of the notice.

Documents to prove identity:

  • U.S. government, military, state, or Tribal issued ID;
  • Military discharge documentation;
  • Weapons permit;
  • Government assistance program document;
  • Statement of benefits from a qualifying program document;
  • Unemployment/worker’s compensation statement of benefits; or
  • Eligibility confirmation from state eligibility database with administrator.

Documents to prove you are alive:

(unexpired and dated within the last 3 months)

  • Current program eligibility or government assistance documentation;
  • Current utility bill (phone and internet bills cannot be used as utility bills; water, electricity, and waste and removal bills can be used as utility bills);
  • Current income statement such as a paystub;
  • Current mortgage or lease statement;
  • Current retirement/pension statement of benefits;
  • Current unemployment/workers’ compensation statement of benefits; or
  • A notarized letter affirming the subscriber’s identity and alive status.

If you do not have any of the documents listed above to prove your identity, please submit one document with your DOB and a second document with your SSN4.

    • Permanent resident or permanent resident alien card; or
    • Passport.

The Affordable Connectivity Program (ACP) is a long-term, broadband federal government benefit program by the Federal Communications Commission (FCC) to eligible households and replaces the Emergency Broadband Benefit Program.
